Help - Creating an Action Plan

 

What are Action Plans?

Action Plans allow you to create a personalized plan of action for each of your clients. Each client Action Plan allows you to record the following high level information:

  1. Start Date - this is the start date for this Action plan, and will be displayed in the table of clients when selecting View Clients, View All Action Plans or View Latest Action Plans
  2. Due Date - this is the due date for this Action Plan
  3. End Date - this is the end date for this Action Plan
  4. Status - options:
    1. Assigned
    2. In Progress
    3. For Approval
    4. Approved
    5. Rejected
    6. Working
    7. Complete 
  5. Notes - this is an open ended text area where you can record anything about this client and the action plan

 

 

 

 

Action Plan Data area

The Action Plan Data tab is where you will record all the details about the presenting issues, goals and services. Each of these sections is described below:

  • Instrument Items -
    • Assessment instruments, incidents and other presenting issues that are recorded in SMR may be selected for inclusion in an Action Plan.
    • Click on the "+" icon to open the instrument selection menu:

    • Select each instrument item to be included in this Action Plan and click on the "Add Answer" icon

    • After clicking on the OK button, you may select a specific instrument item to provide further detail.

  • Goals
    • Defining goals for this Action Plan allow you to define the specific goals that this Action Plan is designed to achieve
    • Goal items are defined by your SMR administrator under the Administration - Taxonomy area.
    • To select a goal for this Action Plan, click on the Goals area to expand the Goals section, then click on the  "+" icon

    • Select a goal by clicking on the "+" next to the Goals data area, then pick a goal from the set.
    • Enter Date, Notes, and Status and click on OK to save this goal to this Action Plan.

 

 

  • Services

    • Defining services for this Action Plan allow you to define the specific services that this Action Plan must deliver to achieve its goals
    • Services items are defined by your SMR administrator under the Administration - Taxonomy area.
    • To select a service for this Action Plan, click on the Services area to expand the Services section, then click on the  "+" icon

    • Select one or more services from the list, then enter any additional information in the Notes section.

Overview

The Overview tab provides an at-a-glance view of the Notes and Data sections of the Action Plan.
