What are Action Plans?

Action Plans allow you to create a personalized plan of action for each of your clients. Each client Action Plan allows you to record the following information:

  1. Start Date
  2. Due Date
  3. End Date
  4. Status
  5. Notes
  6. Data

          i. Assessment Items
          ii. Goals
          iii. Service

7.Progress Notes

 

 

 

Practical Issues

In practice, you will want to establish an action plan strategy that addresses the following:

a. What criteria must be met to initiate an Action Plan?

For example, a threshold level of high risk assessment scores, incidents, or other presenting issues have been recorded elsewhere in the system.

 

b. What is the frequency and standard duration of an Action Plan?

For example, an Action Plan will be done quarterly for a clients who meet the above criteria, and will have a duration of 3 months.

 

c. How often will the Action Plan be updated with Progress Notes?

For example, a Progress Note will be added to each Action Plan every Friday.

 

 

Getting Started

The system provides you with a high level of functionality to manage, view and edit the Action Plans for which you are responsible. 

Action Plans are accessible through the system using the top menu bar under "Measurement Tools".

 

The Action Plan navigation options include:

1. View all clients

2. View all Action Plans

3. View Latest Action Plans, which shows the most recent Action Plan for each client.

 

 

To create an Action Plan for a client, double-click on the client's name from the View All clients option:

Then, click on the New Action Plan icon for the selected client:

You will be presented with a blank Action Plan which you can populate as described in the Create an Action Plan help page.
