What are Incidents?

Incidents are actions involving clients and/or staff that result in harm or damage. The following information about Incidents can be captured using SMR:

  1. Date
  2. Location
  3. Severity
  4. Severity Type
  5. Related Party/Parties
  6. Description
  7. Disposition
  8. Administration Follow Up 

 

Getting Started

SMR provides you with a high level of functionality to manage, view and edit Incidents 

Incidents are accessed through SMR using the left-side navigation Incidents bar.

 

The Incidents options include:

1. View all clients

2. View all Action Plans

3. View Latest Incidents, which shows the most recent Incidents

 

 

To create an Incident for a client, double-click on the client's name from the View Clients option:

Then, click on the Create New Incident icon for the selected client:

You will be presented with a blank Incident worksheet which you can populate as described in the Create an Incident help page.
