1 What are Action Plans?

3 Action Plans allow you to create a personalized plan of action for each of your clients. Each client Action Plan allows you to record the following high level information:

5 1. Start Date - this is the start date for this Action plan, and will be displayed in the table of clients when selecting View Clients, View All Action Plans or View Latest Action Plans
6 1. Due Date - this is the due date for this Action Plan
7 1. End Date - this is the end date for this Action Plan
8 1. Status - options:

11 ii. In Progress
12 iii. For Approval
13 iv. Approved
14 v. Rejected
15 vi. Working
16 vii. Complete

18 5. Notes - this is an open ended text area where you can record anything about this client and the action plan

26 The Action Plan Data tab is where you will record all the details about the presenting issues, goals and services. Each of these sections is described below:

33 ** Assessment instruments, incidents and other presenting issues that are recorded in SMR may be selected for inclusion in an Action Plan.
34 ** Click on the "+" icon to open the instrument selection menu:

53 ** Defining goals for this Action Plan allow you to define the specific goals that this Action Plan is designed to achieve
54 ** Goal items are defined by your SMR administrator under the Administration - Taxonomy area.
55 ** To select a goal for this Action Plan, click on the Goals area to expand the Goals section, then click on the "+" icon

70 ** Defining services for this Action Plan allow you to define the specific services that this Action Plan must deliver to achieve its goals
71 ** Services items are defined by your SMR administrator under the Administration - Taxonomy area.
